Health promotion Services work with individuals, families and communities to create opportunities for people living with disabilities.
We provide skilled and timely care to our participants while promoting their independence and involvement in care decision-making.
We also work collaboratively with allied health teams to set up a program that will support participants for long-term success.
A registered nurse (RN) provides oversight for one hour every day and is contactable after hours to support the team in delivering positive health outcomes.
We support clients with multiple and complex needs such as:

Complex seizure management
When our clients (many of whom are non-verbal) require a visit to the hospital, our staff escort them to provide reassurance and to advocate on their behalf. We have achieved great success with the program, with no clients requiring long-term readmission.
We’re keen to assist more people who require higher level care to move out of the hospital environment and start living a more independent life with reliable nursing and disability support
